Now. I don't claim I understand Dark Souls. After all I never finished the game. But I personally understand it as an After The End story where world is already done for. There doesn't seems to be any civilisation left, only ruins, some better preserved than others, filled with aggressive monsters and few scattered individuals still clinging to sapience (i.e. those you can talk to).
The thing is that its a cycle, so it keeps happening every couple of centuries, it was technically the second time in Dark Souls, but worst than the first.
Civilization keeps reforming over the ruins of the old ones because the fire is reignited all the time. Its also possible that the old civilizations don't even really fall permanently, time is convoluted and shit.
Ah, regarding Dark Souls II, we're not really certain how far the devastation of the outside world has progressed. It's strongly implied that the Drangleic we see outside of the opening cinematic is heavily time-fucked, even before you start delving into the memories of the dead, so we can't really rely on what little news its denizens supply of elsewhere.
I don't know. I understood the lack of direction as an suggestion there isn't anyone to give you directions left in the first place. On the other hand, whole rekindling the fire sacrifice would be rather moot as there won't be anyone to save anymore.
The world relies, runs on Fire. When everything's rekindled, think of it kinda like a restart. Time lines up again ( presently, y'know how finicky time stuff is ) and people hollowing stop, with the Dark Sign fading until the Dark creeps back.
Evdryones happy and all the things are nice! Kingdoms pop up celebrating surviving and Onion Knights are cool again!
