I looked up the term, and IIRC it meant 'insecurity about having small tits', which is distinct from just having them. AIUI, you don't even need to have a character with small tits to tag pettanko, they just need to be smaller than another character's.

Given that it's a Japanese term, I wouldn't be surprised if English usage of it usually doesn't carry the nuance, especially when the presentation is 'character focused on their flat chest'. Double-checking, I myself got it slightly wrong, as it's 'insecurity or obsession', so a character who's very proud of their small tits would also count.

Worse, it's a Japanese slang term - the meaning has probably mutated multiple times since it started to be used by the Anime community.
I wouldn't be surprised to discover that's one of the terms different anime groups disagree about its meaning.

Edit: I've since checked everything against your Synonym Groups threadmark and removed the points it already covers. Two points are withdrawn after lurkingkraken's reply (struck through below).
1. Typos / formatting
- {... eventual oyakodon - possible oyakodon} oyakodan → canonical should be "oyakodon" (that tag exists, 9).
- {mild bimbofication} bimboification → "bimbofication" exists (45); "bimboification" looks like a typo.
- {womb tattoos} womb tattoos → points at itself. Probably meant {womb tattoo} womb tattoos (both exist, 6 each).
- {violent bloody deaths} violent death. → the canonical has a trailing period; should be "violent death".
- {semi-forced prostitution} forced_prostitution → the canonical has an underscore; should be "forced prostitution" (you merge other underscore tags into the spaced version, e.g. face_fucking → face fucking).

Most of the time the "it's not rape if…" tags and statements are thin self-justifications for the character committing rape to not feel guilty about committing rape.
And given that that one work also used the tag 'rape as a battle tactic', it most likely is a case of rape.
While technically true, the vast vast majority of the time ahego is caused by being fucked silly and having mind shattering orgasms.
If this were an image board, or a video site, that separation would matter because sometimes people do make that face for meme purposes, but in stories it's a fine synonym because people looking for ahego stories are looking for stories of people who have a massive orgasm.
